CLINIC
North Simcoe Health and Wellness is located in Barrie, Ontario. Our team of physicians are members of the Barrie Community Family Health Team (BCFHT) and Barrie Family Health Organization (FHO).

We offer various types of appointments.

SAME-DAY
-
​Call in on the morning if you would like an appointment. Phones are answered starting at 8:30am on weekdays. It is advisable to call early in the morning as the day may fill up quickly.

PRE-BOOKED
-
A portion of each day is allowed to be booked in advance (usually up to 15 appointment slots per day). Many of these slots are used for routine follow up and chronic care visits, but are also available to patients who wish to pre-book. These spaces are limited and may need to be booked weeks in advance.

AFTER-HOURS

Our physicians co-own certain walk in clinics associated with the Barrie FHO. If you are a rostered patient at North Simcoe Health and Wellness, you may attend any of our walk in clinics after hours and be seen by the physician on duty. Our after hours clinics can be found on www.barriefho.ca Please try to use these clinics for urgent care needs only. For chronic issues or test results it is best to see one’s own regular primary care provider.
MEET OUR TEAM

Dr. Sumit Makkar
M.B.B.S, MD, CCFP
Dr. Makkar is a trusted family physician offering comprehensive primary care to patients in the Barrie community. He completed his undergraduate studies in Pre-Health at the University of Waterloo before earning his medical degree (MBBS) from Kasturba Medical College in Manipal, India. He later completed his Family Medicine Residency at the Northern Ontario School of Medicine in Sudbury. Since joining the Barrie medical community in 2017, Dr. Makkar has been an active leader in local healthcare. He currently serves as a Director for the Barrie Family Health Team/Family Health Organization, Co-Medical Director of the Barrie and Community Family Medicine Clinics, and a Medical Examiner for the Medical Council of Canada.
In addition to his family practice, Dr. Makkar is also a physician at the MWMCC Weight Management Clinic, where he supports patients in achieving healthier lifestyles. He has a strong passion for obesity medicine and is committed to helping individuals reach their wellness goals through compassionate, evidence-based care. When he’s not in the clinic, Dr. Makkar enjoys spending time with his family, travelling, and creating lasting memories.

Dr. Tom Yokogawa
MD, CCFP
Dr. Yokogawa
completed his undergraduate, masters and medical degree at the University of British Columbia. He subsequently completed a 2 years postgraduate residency in Family Medicine at the Northern Ontario School of Medicine.
Upon completion of his training, Dr. Yokogawa practiced urgent care and hospitalist medicine in Sudbury, Ontario. He moved to Barrie in 2018 taking over the practice of Dr. Susan Smith. When not at the office you can find Dr. Yokogawa keeping up with the latest sports, travelling and spending time with his wife and son.

Dr. Diljaan Natt
M.B.B.S, MD, CCFP, AAFP
Dr. Natt is a family physician who relocated to Barrie taking over Dr. Zamanian in December 2019. Dr. Natt completed his Bachelor of Medicine and Bachelor of Surgery degree at Kasturba Medical College, Manipal. He subsequently attained his certification in Family Medicine from St Johns Hospital Detroit, Michigan. He followed this with his Canadian CCFP designation. He is a Director of the Barrie FHT & FHO and also Co Medical Director of the Barrie and Community Family Medicine Clinics. When not in the office you can find Dr. Natt enjoying the Barrie community with his family.

Nurse Practitioner
Erin Matte
RN(EC), MScN
Erin followed in the footsteps of her mother and many other family members when she chose a career in nursing. Erin graduated from York University in 2005 with her Bachelors of Science in Nursing degree. Her nursing experience includes: cardiology, cardiovascular surgery, geriatrics and community care. In 2011, she returned to York University to complete her Masters degree and Primary Care Nurse Practitioner Certificate graduating with the highest academic achievement award. Erin has been working with the BCFHT as a Nurse Practitioner since 2014 and supports the practices of Dr. Makkar and Dr. Yokogawa. Erin is able to independently care for people of all ages. she's able to assess, diagnosis, investigate, manage and refer as deemed appropriate to specialists. She has a special interest in woman's health. Erin lives in Barrie with her husband and two children

Sheena
RN, RNAO
Sheena brings over 20 years of nursing experience to North Simcoe Health, with a specialized background in obstetrics, prenatal, and pediatric care. She is deeply passionate about supporting the health and well-being of women and children, and her warm, knowledgeable approach makes her a valued member of our care team. Sheena remains committed to clinical excellence by regularly attending medical conferences and continuing education courses. She serves as a key resource in our office for routine immunizations, including guidance on alternative vaccine scheduling. Outside the clinic, Sheena stays just as active—spending time with her three energetic children, camping, boating, running, and always chasing her next adventure.

Andrew Hamilton
BScPA
Andrew is a dedicated Physician Assistant who brings a unique perspective to patient care, having previously practiced as a Chiropractor. Originally from Kirkland Lake, Andrew is known for his compassionate, patient-centered approach and strong clinical experience in both emergency medicine and family practice settings. His diverse background allows him to provide well-rounded, evidence-based care, with a focus on clear communication and building trust with each patient he sees. Outside of the clinic, Andrew enjoys golfing, camping, hockey, and taking on DIY home improvement projects.

Lorie
Medical Office Admin Diploma
With over 30 years of experience in medical administration, Lorie is a cornerstone of the North Simcoe Health team. She is known for her professionalism, dedication, and unwavering commitment to ensuring patients have seamless and timely access to care. Lorie has played a key role in the growth and success of our clinic, helping to shape an environment where both patients and staff feel supported. Her focus on maintaining high standards makes her an essential part of our day-to-day operations.
Outside of work, Lorie enjoys spending quality time with her family, reading, traveling, and pampering her beloved Maine Coon cat, Gus.

Karen has been with North Simcoe Health and Wellness since 2021, She has experience in mental health and medical office admin. Her compassion, friendly demeanor and willingness to help always make patients experience pleasant. When Karen is not in the office you can find her spending time with her friends, family and pet snake.
